Breast Cancer Charity Overstates Value of Mammograms: Researchers

THURSDAY, Aug. 2 (HealthDay News) -- The breast cancer charity that brought the world the pink ribbon exaggerates the benefits of mammography while minimizing its harms, researchers claim.

The organization in question is the Susan G. Komen for the Cure, which stated in a 2011 advertisement that the five-year survival rate for breast cancer when caught early is 98 percent, while it is only 23 percent if not caught early.

"The survival statistics they present are eye-catching and compelling. They imply that a woman would be crazy and irresponsible if they didn't go for screening," said Dr. Steve Woloshin, co-author of the article challenging the charity. "But the statistics are deceptive."

According to Woloshin's commentary, which appears online Aug. 2 in the BMJ, a woman in her 50s who goes for regular mammograms for 10 years will only cut her chance of dying by a fraction of a percentage point -- for every 10,000 women who are screened 7 deaths will be prevented.

The reason for the discrepancy?

Survival statistics calculate how long a woman lives only after diagnosis, the researchers explained.

If 100 women were diagnosed with breast cancer after feeling a lump at age 67 and all died three years later at the age of 70, the five-year survival rate would be 0 percent.

But, if the cancers were detected by mammography when the women were 64 and they still died at the age of 70, the five-year survival would be 100 percent.

Komen also minimizes the harms that can come from over-screening, according to the article.

For every woman whose life is saved by mammography, between two and 10 women are overdiagnosed, meaning they are told they have cancer when they do not and end up going through unnecessary treatment.

And up to half of women who are screened every year for a decade receive at least one false positive, meaning they have to undergo a biopsy and experience the fear of thinking they have breast cancer, if only temporarily.

The article comes at a time when there has been increasing furor over the value of breast cancer screening. The U.S. Preventive Services Task Force now recommends that women in their 40s do not get regular mammograms.

In contrast, the American Cancer Society recommends that all women aged 40 and over get annual mammograms.

In the end, women need to get reliable information from their physicians or other sources about the risks and benefits of mammography. One problem is that not all primary care physicians know the right numbers to convey to their patients, according to a recent survey.

Overall, few doctors would argue that there isn't some benefit to mammography.

Dr. Stephanie Bernik, chief of surgical oncology at Lenox Hill Hospital in New York City, supports the charity's push for cancer screening.

"As physicians, we feel that breast cancer screening is extremely beneficial," she said. "Screening does save lives, but it's probably not as dramatic as it's sometimes made out to be."

Adds Woloshin, who is professor of medicine and community and family medicine at Dartmouth School of Medicine and The Dartmouth Institute for Health Policy and Clinical Practice: "There is some benefit but the magnitude is of a different order from what is implied by the ad. Women should make good decisions for themselves."

A representative of the Komen Foundation defended the organization's position.

"Everyone agrees that mammography isn't perfect, but it's the best widely available detection tool that we have today," said Chandini Portteus, Komen's vice president of research, evaluation and scientific programs. "We've said for years that science has to do better, which is why Komen is putting millions of dollars into research to detect breast cancer before symptoms start, through biomarkers, for example," she explained.

"Komen also is funding research to help accurately predict which tumors will spread and which won't," Portteus continued. "While we invest in getting those answers, we think it's simply irresponsible to effectively discourage women from taking steps to know what's going on with their health," she noted.

"The numbers are not in question," Portteus said. "Early detection allows for early treatment, which gives women the best chance of surviving breast cancer."

French town famed for extreme sports bans wingsuit

PARIS (AP) ? The French town of Chamonix, deep in the shadow of Mont Blanc, has always embraced danger.

Climbers scale Europe's highest peak in dire conditions. Backcountry skiers risk avalanches or falling off cliffs.

But the arrival this summer of the wingsuit flyers offered peril on an entirely new scale. For nearly two months, daredevils in skin-tight suits with batwing sleeves and a flap between their legs hurled themselves off the Brevent cliff, soaring through the Alpine skies. Last week, tragedy struck: A Norwegian wingsuit flyer was killed when his parachute failed to open.

The next day, the mayor of Chamonix-Mont Blanc banned wingsuits.

The decision has triggered a debate about how to weigh the dangers of extreme sport against the passion of the thrill-seekers the Alpine town has famously encouraged. The ban isn't meant to be permanent ? local officials hope to come up with a set of rules on wingsuits that will satisfy everyone. But Chamonix remains shaken.

"For us, adventure doesn't mean extreme risk," said Chamonix mayor Eric Fournier. "We have to ask questions of responsibility and respect for other sports."

In Chamonix, near the Swiss and Italian borders, paragliders and hikers share space with mountain gondolas, climbers and BASE jumpers, adventurers who leap from high places and float down by parachute.

Last month, nine experienced climbers died in an avalanche on the French side of the mountain. And there have been increasing concerns that the area is becoming just too crowded.

Wingsuits are a relatively new offshoot of BASE jumping, coming into their own only in the late 1990s. Flyers start high and hope for a breeze strong enough to make the flight last, before opening their chutes to land.

Mont Blanc winters are long and harsh, and the wingsuit season there is short, dependent upon warm air and favorable conditions for landing.

That's where the cliffs above Chamonix come in. Popular in winter among skiiers, in summer among paragliders and hikers, the cliffs are easily accessible by trail. That's just high enough to allow for a wingsuit jump and close enough for multiple jumps in a day ? a rarity in the wingsuit world. As a wingsuit site, Brevent was relatively unknown until late this spring, when YouTube videos caught the attention of flyers everywhere.

They came by the dozens, according to Fournier and Roche Malnuit, who lives in Chamonix and is president of the French BASE Association.

It's an exhilarating, dangerous sport with a small number of passionate adherents, numbering a couple hundred in France and perhaps a thousand or two in the world. The suits can cost over ?1,000 ($1,200) and are made from the same synthetics used for hot-air balloons or parachutes. Flyers commonly jump from planes or helicopters ? less commonly from cliffs ? and can soar long distances before opening their parachutes to land.

The world record, set from an airplane jump, stands at 16 miles. For BASE jumping wingsuit flyers, the distance is considerably shorter, Malnnuit said. At Brevent, where the cliff's stands at an altitude of 2,500 meters (7,500 feet), Malnuit said flyers can expect to travel about 4.5 kilometers (nearly 3 miles).

Malnuit learned to fly in a wingsuit from his father, who was a BASE jumping pioneer in the 1980s. Malnuit said his father gave it up at age 50 "because the risks became too great. He said he was happy to have lived it."

"I love nature, I love sports, I love the mountains," Malnuit said, explaining his own path to wingsuit flight. "I had my father who was there to teach me. He taught me that it's a sport of risk."

Tore Hovda is the brother of 38-year-old Jon Inge Hovda, who was killed after jumping from the Brevent cliff on July 23.

Hovda said his brother got into the sport 15 years ago after starting out with parachuting. "That (BASE jumping) was his whole life. He just couldn't stop with it."

Despite his brother's death, Hovda said he did not think the sport should be forbidden.

"People should be able to jump if they want to," Hovda said from his home in Stavanger, in southeastern Norway. "It would be wrong to put a ban on it."

Many public places have done just that, including national parks in the United States. Even in California's Yosemite, where solo rock climbing without safety gear is permitted and considered a mark of the highest skill, wingsuits are banned along with other BASE jumping, for many of the same reasons Chamonix is struggling with the sport.

"Yes, we allow free soloing and rock climbing, which are also high risk activities. However, the ban on BASE Jumping is not based on the high risk nature of the activity, rather it is based on its overall impacts to other park activities: the 'circus-like' atmosphere it creates, its impacts on climbers, potential impacts to park resources, etc." Scott Gediman, a spokesman for Yosemite, told The Associated Press in an email. Yosemite has dozens of climbers a day at the high season and thousands of visitors on the trails.

Gediman said he himself watched a BASE jumper leap to her death in 1999 when her borrowed chute failed to open.

Fournier, whose sports include skiing, paragliding and mountain climbing, doesn't envision a permanent wingsuit ban in Chamonix. He doesn't want to try wingsuits, but he understands the appeal and he expects to work with flyers to figure out how to proceed.

"It's a practice that taxes the mind and the body," said Fournier. "In these last few weeks, it was opened too quickly to too many people."

Malnuit supported time to reflect on the dangers, but said he hoped the cliffs will reopen soon to wingsuits.

"With wings, you truly arrive at flying," he said. "You can steer, you can accelerate. You're truly in flight."

J.J. Hunsecker (Burt Lancaster)? understands the importance of ?theme? (The Sweet Smell of Success).

I find it hard not to think of screenwriting ? or for that matter, writing in general ? within the context of my journalism schooling. Old habits die hard, and they also have a way of defining new habits.

While teaching an entry-level news writing course, one of my college journalism professors described two kinds of news writers: Those who can get past their lede, and those who can?t.

The lede ? journo-speak for ?lead? ? simply refers to the opening paragraph of a news story. Ideally, your lede delivers the most bare-bones, essential version of the facts while establishing both the?Raison d??tre?and tone for the rest of the story. Without an intriguing (or in the very least, sound) lede, you?ve lost your reader from the get-go.

The average morning commuter reading the paper before work doesn?t have time to be confused. He or she wants a clear communication of what they?re getting into. A jumbled lede says, ?Move along, we?re still sorting things out here? rather than ?This is what happened, see below for details?.

As my professor observed, some writers can skip over their lede ? leaving a placeholder or nothing at all. They write the entire story and go back to the beginning, retroactively distilling the information of the piece down to a lede.

And the others? well, I was in that camp. The news writers who knew no shoddy temporary lede would suffice. In order to continue writing, I always had to have my lede more or less perfect, in close-to-finished form. Or else, no further work could continue.

The analogous ?lede? for the screenwriter would logically be the opening of the script; Page 1, Scene 1, ?We begin in darkness.?* However in my ongoing campaign to eschew logic, I?d like to suggest the titling of a script is much more akin to writing a ?lede?, and in many ways, just as important.

*Please, for love of the written word, never begin a screenplay with ?We begin in darkness? or any other variation on that all-in-it-together POV.? I know there are different schools of thought on whether a (spec) script should ever implicitly include the reader/audience with references to ?We see? or ?We hear?, but I?m firmly against it.

Find a way to write the image or the sound. ?Darkness.? establishes everything ?We begin in darkness.? does while being more concise and evocative.

Francis Ford Coppola has talked about the importance of boiling down the central theme of a film (or for our purposes, screenplay) to one or two words. In times of uncertainty, this one or two-word focal point serves as a guide to the director (or for our purposes, writer). Every decision comes back to the theme. What is the film (script) about?

When I write, or even when I plan to write (outlining, note taking, etc.), I cannot work unless I have a title in mind. It may change, but I need a title and I need to know I at least have the intention of keeping/using the title. This is some serious cart-before-the-horse action when you?re talking about taking notes on an idea I may never follow through on.

The short answer is, hey, I?m a bit OCD when it comes to certain things. But if you?ll allow me a longer rationalization of my behavior, I think of the title like my lede. A script or movie title, by nature, will probably be a little more oblique than one of Coppola?s one or two-word thematic distillations.

For example, in the linked article above, Coppola mentions The Conversation, for which he summed up a film about ?privacy.? The Conversation alludes to the central plot of the film ? private investigator Harry Caul discovers a sinister plot while listening in on tapes he recorded for a corporate executive. There?s a suggestiveness to the simple title, which turns out to be ironic given Caul?s life in near-isolation.

Take a look at Sweet Smell of Success, a title that cuts right to the heart of the film: Mocking, sordid, both petty and grand. The sing-song alliteration even mimics them rhythmic flow of the script?s cracker-jack dialog.

Titles are promotional tools as much as they are true reflections of the works they?re attached to. At the writing stage however, a title should be your lede. Setting the tone, creating a hook or providing an answer for the questions that will follow. A place the writer can revisit to remember why he or she even started writing the piece in the first place.

Jennifer Lopez and Casper Smart Threaten Legal Action Over Tabloid Cover Stories


Casper Smart frequent exotic massage parlors and/or is a homosexual seeking fast and easy one-night stands.

So claim a pair of tabloid cover stories this week, both of which have prompted Jennifer Lopez and her boyfriend to threaten lawsuits against the publications behind them.

The first, from In Touch, quotes a couple of employees at a massage parlor in New York City who allege Smart has stopped by for appointments there twice in the last month.

Star, meanwhile, asserts that the dancer was caught at a peep show in the Big Apple last month, somewhere that's a "well-known cruising spot for gay men looking for instant hookups."

According to TMZ, the couple's attorneys, Shawn Holley and Howard Weitzman, have sent letters to both magazine warning that the "reports" are "false, malicious and defamatory" and will cause "immense damage" to Jennifer and Casper's careers and reputations. They pair will follow up with legal action unless retractions are printed.

No response so far from In Touch or Star.

Lions GM surprised Best hasn't been cleared yet

ALLEN PARK, Mich. (AP) - Detroit Lions general manager Martin Mayhew prides himself on not sharing too much information.

He laughs at reporters who try to exchange more than pleasantries with him throughout the year when it's not one of the rare days he makes himself available to the media.

However, Mayhew did take and answer questions on Tuesday morning and couldn't hide his feelings about waiting for Jahvid Best to be cleared for practice.

"I'm a little bit surprised, a little bit disappointed," Mayhew acknowledged. "Everything was trending toward him being ready to go the first day of training camp."

Best had a season-ending concussion on Oct. 16, 2011 - his second of the year and at least his third overall - and seemed to be making progress during the offseason.

Like the Lions, Best isn't pleased that he's being held out by doctors despite not having any obvious concussion symptoms.

"It's definitely frustrating," he said.

Detroit is counting on Best to help strike a balance with its stellar passing game led by quarterback Matthew Stafford and receiver Calvin Johnson.

The Lions are also banking on defensive end Cliff Avril reporting to camp to sign a $10.6 million, one-year deal after he and the team couldn't agree on a long-term contract.

"Eventually, I know he will be here," Mayhew said. "And he'll be in great condition when he gets here."

Messages left with Avril, seeking the planned date of his arrival in Allen Park, haven't been returned.

"If anything is positive or encouraging to me, he's very optimistic about how he's going to perform this season," Mayhew said. "He has his reasons for not being here. We made a very solid offer to him that we felt was a really good offer. He was willing to bet on himself and bet that he's going outperform what was on the table, and I like that."

The Lions, ranked No. 11 in the inaugural AP Pro32 NFL power rankings, are coming off their first appearance since the 1999 season. Mayhew said he thinks this team is better than the last one, but he's not willing to say returning to the postseason is an expectation of his.

"It's important for us to not start focusing on the playoffs right now," he said. "My focus right now is to have a good practice today and I think a lot of our teams feels that way. I know Jim (Schwartz) preaches that. The only way you get back to the playoffs is by taking care of the steps along the way."

Center Dominic Raiola has endured a lot of losing since Detroit drafted him in 2001, including the NFL's only 0-16 season just four years ago. He said the Mayhew-led front office deserves a lot of credit for turning the franchise around.

"If you look at the talent that we're bringing in and keeping, it's night and day," Raiola said. "They're giving us everything it takes to win."

Detroit is missing only one starter from last season, cornerback Eric Wright, who signed with Tampa Bay as a free agent, and he left a void that Aaron Berry was projected to fill this year. However, Berry had his contract terminated last week after being arrested twice this summer. That leaves the Lions with lackluster options - Alphonso Smith, Jacob Lacey, Justin Miller along with rookies Dwight Bentley and Jonte Green - to start opposite cornerback Chris Houston.

"My belief in the spring was that we had a very heated competition at the right cornerback spot," Mayhew said. "There were a number of players involved and a lot of those guys are still here competing."

Mayhew said he would be willing to add a veteran running back or cornerback at some point, but believes it is too early to make such a move.

So, what is his biggest concern a little more than a month before opening the season at home against the St. Louis Rams?

"Not saying anything silly right now in talking to you guys," Mayhew said.
